Job summary

Bonifay-Nursing-and-Rehab-Center is seeking a Graduate Practical Nurse (GPN) to provide care under physician orders and RNs. The GPN will work in a team and must obtain LPN licensure within 30 days of employment.

The incumbent will collect data, observe residents, administer medications, and contribute to the interdisciplinary care plan while documenting all actions per policy and standards.

Qualifications

• GPN must complete licensure as LPN within 30 days of employment.
• Collaborates with RNs and physicians for care planning.
• Follows standards of care and company policy when documenting observations.

Responsibilities

• Collect resident data such as vital signs and complaints.
• Observe residents and document treatment responses and status.
• Administer and document medications and treatments as ordered.
• Assist interdisciplinary care plan team in developing plan of care.
• Communicate relevant observations to RNs/physicians.
• Document observations and care in compliance with standards and policy.
• Complete required forms and records per regulations and policy.

Job description

A graduate practical nurse (GPN) provides care and treatment in accordance with physician orders. Works in a team environment and within scope of practice as defined by State. The GPN works under the direction of physicians and registered nurses (RNs). The GPN is expected to obtain LPN licensure within 30 days of employment.

• Collection of data (may be part of an evaluation (e.g., vital signs, resident’s complaints, symptoms, etc.)
• Observation of the residents (e.g., location, activity, position, etc.)
• Administers and documents medications and treatments
• Collection of information to assist the interdisciplinary care plan team in the plan of care
• Communicates with RNs and or physicians regarding pertinent resident observations and information
• Documents observations and care in compliance with standards of care and company policy
• Completes required forms and documents in accordance with company policy and state and/or federal regulations
