Job summary

Princeton Health and Rehabilitation is seeking an RN/LPN to render nursing care in assigned areas, under the Unit Manager, in alignment with established standards. The role focuses on individualized patient care and nursing procedures within a rehabilitative setting.

The position requires graduation from an RN/LPN program and a current license, with duties including medication administration, IV therapy, and care plan coordination.

Qualifications

  • High School Diploma or Equivalent.
  • Graduate of an accredited RN/LPN program.
  • Valid Graduate State RN/LPN license.

Responsibilities

  • Utilize nursing knowledge and skills in safe basic preventive, therapeutic and rehabilitative nursing care for assigned customers.
  • Initiate and lead team conferences in development of individualized nursing care plans.
  • Assist with or institute emergency measures for sudden adverse developments in customers.
  • Contribute to development of the customer/significant other plan of care based on identified issues.
  • Assist in evaluation of effectiveness of nursing care relative to identified needs.
  • Document pertinent observations and interventions in the EMR.
  • Perform selected tasks related to customer care and the environment safely.
  • Administer medication accurately, observing customer response and documentation.
  • Perform IV therapy procedures for peripherally inserted IV fluids, per RN scope of practice.
  • Coordinate communication among customers, family, team members, and physicians as reflected in EMR and feedback.
  • Assist in ensuring effective communication with customer/family as reflected in plan of care.
  • Provide customer/family health teaching as indicated by condition and need.
  • Utilize teaching methods such as one-on-one and group instruction; document responses.

Education

The RN/LPN is responsible to the Unit Manager and its clients for rendering nursing care in assigned area in terms of individualized customer needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse.

Essential Duties and Responsibilities
  1. Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned customer/family as evidenced by documentation and observation of positive customer care outcomes
  2. Initiates and leads team conferences in development of individualized nursing care plans
  3. Assists with or institutes emergency measures for sudden adverse developments in customers
  4. Contribute to the development of the customer/significant other plan of care based on the identified issues, directly and indirectly
  5. Assist in the evaluation of the effectiveness of the nursing care administered relative to the customer/ significant other identified problem/needs
  6. Document pertinent observation and intervention as appropriate in the EMR
  7. Perform selected tasks related to customer care and the customer's environment in a safe manner
  8. Administer medication accurately, observing customer response, as evidenced by documentation in the EMR and lack of negative outcomes
  9. Perform I.V. Therapy procedures on those customers receiving peripherally inserted Intravenous fluids, pursuant to RN scope of practice.
  10. Maintain and coordinate communication among customer/significant other, team members, and physician as evidenced within the EMR, client, and health care team feedback.
    • Assist in assuring that effective communication exists with customer/significant other as reflected in the plan of care
    • Give punctual change of shift report consisting of pertinent and relevant information for assigned customers
    • Effectively communicate customer’s changing needs and plan of care among team members
    • Report customer needs/problems and observations regarding customer’s condition to Unit Manager, ARNP, and Physician
    • Reinforce customer/significant other information regarding plan of care and discharge plans
    • Work collaboratively with other members of the health care team in coordination of the customer's care as evidenced by other department feedback
  11. Provide customer/significant other health teaching as indicated by condition and need
  12. Use available written and AV materials specific to customer condition or disease
  13. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussion
  14. Document health teaching including customer/significant other response to and their understanding of health teaching
  15. Participate in staff development programs as evidenced by education record
  16. Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate resources
  17. Comply with the infection control policies and procedures including ensuring a safe working environment for self and others
  18. Attends and participates in in-service training.
Required Education and Experience
  • High School Diploma or Equivalent
  • Graduate of an accredited RN/LPN program
Required License/Certification
  • Valid Graduate State RN/LPN license
  • Current CPR certification
