Job summary

PureHealth Transitional Care at THR Arlington seeks a Licensed Practical Nurse (LPN) to deliver high-quality nursing care in a post-acute setting, reporting to the Unit Manager and working under RN supervision.

Shifts are 6a-6p or 6p-6a, with emphasis on documentation in the EMR, medication administration, and collaborative care planning to ensure safe, effective patient outcomes.

Qualifications

  • High school diploma or GED required; LPN program graduate required.
  • CPR certification required.
  • Licensed Practical Nurse in good standing.
  • Experience in post-acute or SNF setting preferred.

Responsibilities

  • Provide nursing care in accordance with the patient care plan and nursing standards.
  • Lead care planning discussions and participate in team conferences.
  • Administer medications and monitor patient responses.
  • Document care interventions and patient status in the EMR.
  • Collaborate with physicians, RNs, and other team members to meet patient needs.
  • Maintain communication with patients and families regarding care plans and discharge plans.
  • Attend in-service trainings and adhere to infection control policies.

Position Summary

The LPN is responsible to the Unit Manager and its clients for rendering nursing care in assigned area in terms of individualized customer needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse.

Essential Duties and Responsibilities
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned customer/family as evidenced by documentation and observation of positive customer care outcomes

  • Initiates and leads team conferences in development of individualized nursing care plans

  • Assists with or institutes emergency measures for sudden adverse developments in customers

  • Contribute to the development of the customer/significant other plan of care based on the identified customer problems/needs

  • Carry out physician orders accurately, directly and indirectly

  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the customer/ significant other identified problem/needs

  • Document pertinent observation and intervention as appropriate in the EMR

  • Perform selected tasks related to customer care and the customer's environment in a safe manner

  • Administer medication accurately, observing customer response, as evidenced by documentation in the EMR and lack of negative outcomes

  • Perform limited V. Therapy procedures on those customers receiving peripherally inserted Intravenous fluids, pursuant to LPN scope of practice.

  • Maintain and coordinate communication among customer/significant other, team members, and physician as evidenced within the EMR, client, and health care team

    • Assist in assuring that effective communication exists with customer/significant other as reflected in the plan of care

    • Give punctual change of shift report consisting of pertinent and relevant information for assigned customers

    • Effectively communicate customer's changing needs and plan of care among team members

    • Report customer needs/problems and observations regarding customer's condition to Unit Manager, ARNP, and Physician

    • Reinforce customer/significant other information regarding plan of care and discharge plans

    • Work collaboratively with other members of the health care team in coordination of the customer's care as evidenced by other department feedback

  • Provide customer/significant other health teaching as indicated by condition and need

  • Use available written and AV materials specific to customer condition or disease

  • Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussion

  • Document health teaching including customer/significant other response to and their understanding of health teaching

  • Participate in staff development programs as evidenced by education record

  • Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate resources

  • Comply with the infection control policies and procedures including ensuring a safe working environment for self and others

  • Attends and participates in in-service

  • Collaborates with N.A. regarding patient's current and change of status

  • Perform IV therapy on patients within LN scope of practice

  • Administers medication accurately, as evidenced by the EMR and lack of negative patient outcomes

  • Takes a proactive approach to checking and stocking supplies

Work Environment

This job operates in a post-acute setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facility's no-smoking policy.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 60 pounds.

Position Type and Expected Hours of Work

This is a position in a post-acute setting that is open 24 hours a day, 365 days a year. Weekend and holiday shifts will be required.

  • Ability to react decisively and swiftly in emergency situations

Required Education and Experience
  • High School Diploma or Equivalent

  • Graduate of an accredited LPN program

Required License/Certification
  • Valid State Appropriate LPN license

  • Current CPR certification

Preferred Education and Experience
  • A minimum of one (1) year of nursing experience in a SNF
