• Collects patient data and completes forms under the supervision of an RN.
• Develops a plan of care with patient/family and other disciplines; referrals as needed under supervision.
• Documents care delivered and observed changes; monitors and reports patient status as required.
• Participates in QI processes and adheres to policy, infection control and patient confidentiality.
