LiveWell Homecare Agency seeks a Graduate Nurse to provide direct patient care, including assessment, planning, implementation, and evaluation, in a homecare setting after completing a nursing program and prior to licensure.

The role requires licensure within 90 days of graduation and involves educating patients and families, developing care plans, and ensuring safety and quality with ongoing professional development.

The Graduate Nurse provides direct patient care activities, including assessment, diagnosis, planning, implementation, and evaluation, within nursing standards after completing a nursing program and prior to licensure. They must obtain licensure within 90 days of graduation to remain employed.

Key Responsibilities

• Educate patients and families on managing illnesses, post-treatment care, and medication administration.
• Utilize critical thinking to assess patients’ physical, psychosocial, and emotional needs.
• Develop and execute individualized care plans collaboratively, promoting patient-centered care.
• Evaluate patient responses and adjust care plans accordingly.
• Participate in activities ensuring patient safety, quality, and regulatory compliance, along with ongoing professional development.
• This is a safety-sensitive role requiring constant alertness and safe work practices.

Qualifications

• Nursing Diploma or Associate’s Degree in Nursing.
• Ability to obtain licensure within 90 days of graduation.

Preferred Qualifications

Experience or additional certifications in nursing are advantageous but not mandatory.
