Job description

The general practice nurse is responsible for ensuring the delivery of safe and effective nursing care to the entire practice population.

Provide routine patient care and assist with procedures as directed by senior nursing staff and GPs.

Deliver evidence-based practice for patients with long-term conditions and manage preventative nursing interventions for all patients.

The post holder will work collaboratively with the whole general practice team and report to the Nurse Manager.

Work in accordance with the Nursing and Midwifery Code of Conduct (NMC).

Main duties of the job
  1. Patient health checks, obtaining appropriate information using a lifestyle questioning approach. Develop management plans with patients where health or potential health problems are identified.
  2. Blood Pressure and Pulse Monitoring, Height, Weight, BMI, Smoking Status, Alcohol Status, Q-risk calculations.
  3. 24-hour BP & ECG Monitoring.
  4. Travel health assessments, administer injections, and provide guidance in accordance with guidelines using PGD/PSD.
  5. Immunisations (adult and child), childhood and adult injections & immunisations as per PGD or PSD.
  6. Interpret pathology test results as per Clinical Protocols & Training.
  7. Therapeutic monitoring, checking compliance with treatments using a holistic patient-centred approach.
  8. Health screening, recognising issues beyond cervical cytology and contraceptive pill checks.
  9. Wound and Ulcer Care, including trauma assessment, dressings, compression, bandaging, and stocking measurements.
  10. Post-operative wound care, clip and suture removal.
  11. Management of chronic disease Patient Health and Chronic Disease Checks as per Clinical Protocols and Training, e.g., Diabetic Foot Checks.
  12. Ear care, including routine ear care and ear irrigation.

Job responsibilities
  1. Repeat of main duties outlined above, emphasizing patient health checks, health promotion, immunisations, wound care, chronic disease management, and collaborative team working.
  2. Support the local nursing team by maintaining standards, supporting CPD, and participating in quality assurance activities.
  3. Support the practice in achieving its QOF targets and ensure compliance with practice policies.

Professional Role

Work effectively as part of a team, support others, delegate appropriately, and support the development of team members. Support the locality lead nurse in operational and clinical standards, participate in audits, maintain professional registration, and engage in CPD. Record patient data accurately and work collaboratively with colleagues. Support operational management tasks as delegated, promote evidence-based practice, and ensure policies are implemented.

Person Specification
Qualifications
  • Nursing Degree Qualification BSc or MSc or working towards
  • Current NMC Registration RGN
  • Membership of a professional body (e.g., RCN)
  • ENB 998 Teaching and assessing in clinical practice (or equivalent)
  • Chronic Disease Management Qualifications (e.g., Diploma, Degree)
Experience
  • Primary care experience and knowledge of general practice operations
  • Skills in cervical cytology, immunisation, ear care, minor surgery
  • Experience managing chronic disease clinics
  • Understanding of GP surgery structure and functioning
