Job summary

This position is also responsible for performing ongoing functions related to member health-related social needs directly or via social service department and community-based organizations. The position responds to inquiries from members and supports the clinicians in the physical and behavioral health care management department with their provider and member activities. This position is also responsible for supporting relationships with community partners, working with community-based organizations, and contributing to the design of community health outreach programs. This position may require field-based work commensurate with initiatives, health campaigns and departmental function.
