Holyoke Medical Center is seeking a Nurse Navigator in Gastroenterology to coordinate patient care across the continuum from pre-operative through post-discharge, ensuring timely, high-quality service.

The RN will review GI procedure orders, arrange medical clearances, educate patients on bowel prep and medication instructions, and collaborate with gastroenterologists, surgery schedulers, anesthesia, and the OR to remove barriers to care.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license within the Commonwealth of Massachusetts.
• Three to five years of clinical-related job experience.

Responsibilities

• Clinical assessment: review GI procedure orders, medical histories, medications, and risks to determine readiness.
• Medical clearance: coordinate with PCPs, cardiologists, pulmonologists, and specialists; track requirements.
• Patient education: contact patients ~two weeks before procedures to review bowel prep, medications, and dietary/transport needs.
• Care coordination: collaborate with gastroenterologists, surgery schedulers, anesthesia, and OR for timely clinical requirements.
• Patient navigation: identify and resolve barriers relating to transportation, preparation, medications to avoid delays.
• Documentation: maintain accurate records and clearance status in Meditech Expanse.
• Quality improvement: support initiatives to reduce cancellations and improve bowel prep compliance and patient satisfaction.

Our Gastroenterology physician practice is seeking a Nurse Navigator to provide comprehensive care coordination for patients throughout the continuum of care. The Nurse Navigator will coordinate and oversee all aspects of patient care by working closely with providers, patients and families pre-operatively, post operatively and post discharge to ensure that patients receive timely, high quality care.

Key Responsibilities

• Clinical Assessment: Review GI procedure orders, medical histories, medications, and clinical risks to determine procedure readiness.
• Medical Clearance: Coordinate necessary clearances with PCPs, cardiologists, pulmonologists, and specialists; track outstanding requirements.
• Patient Education: Contact patients approximately two weeks before procedures to review bowel preparation, medication instructions, dietary restrictions, and transportation requirements.
• Care Coordination: Collaborate with gastroenterologists, surgery schedulers, anesthesia, and the OR to ensure timely completion of clinical requirements.
• Patient Navigation: Identify and resolve barriers involving transportation, preparation, medications, and other concerns that may delay care.
• Documentation: Maintain accurate clinical records, patient communications, and clearance status in Meditech Expanse.
• Quality Improvement: Support initiatives to reduce preventable cancellations, improve bowel preparation compliance, and enhance patient satisfaction.
• Performs other duties as required

Qualifications

• Education: Graduate of an accredited school of nursing, Bachelor’s degree preferred
• Licenses/Registrations: Current RN license within the Commonwealth of Massachusetts; Valid Massachusetts Driver’s License
• Experience: Three to five years of clinical-related job experience
• Special Aptitudes & Activities: Strong written, verbal, and interpersonal communication skills with the ability to build effective working relationships with patients, providers, staff, and internal and external partners. Demonstrates strong organizational and prioritization skills, attention to detail, and the ability to work independently while effectively collaborating with a team. Ability to understand practice workflows, exercise sound independent judgment within established policies and procedures, and travel between various work settings as needed.
