Job summary

Starr Regional Medical Center in Athens, TN is seeking a per diem Social Worker in Geropsychology to support patient care and discharge planning. This role focuses on coordinating services for patients and families while collaborating with the care team.

Applicants should have a Bachelor's in Social Work, a current TN social work license (LCSW preferred), and a required BLS certification at hire. This position emphasizes compassionate, multidisciplinary care within Lifepoint Health.

Qualifications

Bachelor's Degree in Social Work and TN licensure are required.
Licensed Clinical Social Worker (LCSW) in TN preferred.
Basic Life Support (BLS) certification required at time of hire.

Responsibilities

Interviews and assesses patients and/or family, caregivers, and/or legal representatives.
Determines, prioritizes, provides and/or arranges needed internal and external services/interventions.
Participates in case reviews to evaluate case management and progress; consults with healthcare team to ensure treatment plan compliance.
Assists with discharge planning and referrals to resources (transportation, housing, healthcare, social/spiritual services).
Supervises and/or trains new staff, students, and interns.
Performs Medicaid screenings as indicated for NHP.
