Qualifications

• Master’s degree in Nursing and active APN license in the state.
• Board certification as NP (FNP/AGNP) or equivalent.
• DEA registration or eligibility and prescriptive authority.
• Minimum 2 years clinical experience in geriatrics/long-term care preferred.
• Strong clinical assessment, diagnostic and treatment planning skills.

Responsibilities

• Perform comprehensive health assessments and develop evidence-based care plans.
• Provide ongoing medical management including orders, diagnostics, and medications.
• Deliver timely interventions to prevent hospitalizations and improve outcomes.
• Collaborate with physicians and an interdisciplinary team in care planning.
• Coordinate transitions of care and ensure continuity across settings.
• Educate residents and families on diagnoses, treatment plans, and preventive care.
