Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics or primary care experience to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers, delivering personalized, coordinated healthcare that enhances quality of life.

APRN/NP or PA license is required; 3 years’ geriatric clinical experience, EHR proficiency, and MS Office skills are essential. Competitive benefits and sign-on are offered within a national care model.

Qualifications

• APRN/NP (FNP/AGNP/AHACNP) or PA license required.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Masters degree or higher.
• EHR experience required.
• Proficiency in Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal agencies for clinical services.
• Prescribe medications and order labs, procedures and consultations.
• Monitor patient response and modify treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
