Dignity Health Medical Group - Arizona is seeking a full-time Geriatrician to join our busy department in Prescott, Arizona. The role focuses on geriatrics, dementia, and palliative care, with collaboration across interdisciplinary teams to enhance care coordination and patient outcomes.

You will engage in patient education, shared decision-making, and dementia screening as a geriatrics consultant. A clinic-based position with potential home visits and four hours of administrative time weekly

Qualifications

• BE/BC in Geriatric Medicine (IM or FP), Neurology, or Hospice & Palliative Medicine (IM or FP).
• Experience or interest in Dementia/Alzheimer's disease population.
• Strong communication and interpersonal skills.
• Valid state medical license as MD or DO.
• Valid DEA certificate.
• Ability to work effectively in a team-based healthcare environment.
• Demonstrate empathy, compassion and respect for older adults and their care partners.

Responsibilities

• Care for geriatrics, dementia and palliative care patients in a busy department.
• Collaborate with interdisciplinary teams and APPs to improve care coordination.
• Engage in patient education and shared decision-making.
• Serve as a geriatrics consultant, including dementia screening and diagnosis.
• Clinic position with possible home visits; 4 hours admin time weekly.
• Manage a daily patient volume of 6-10 patients.

Dignity Health Medical Group - Arizona is seeking a full-time Geriatrician to join our busy department in Prescott, Arizona

• Patient population includes Geriatrics, Dementia, and Palliative Care
• Collaborate closely with interdisciplinary teams and Advance Practice Providers to enhance efficiency and care coordination to ensure comprehensive patient care
• Actively engage in patient education and shared decision-making to empower older adults and improve health outcomes
• Serve as a Geriatrics Consultant, including screening and diagnosing Dementia
• Clinic position with possibility of home visits
• Full time position, Monday through Friday
• Provider will have 4 hours of administrative time per week
• Daily patient volume: anticipated 6-10 patients per day
• Current programming includes multi-disciplinary Dementia Screening Clinic, Alzheimer's & Dementia Care Program (UCLA dissemination site), Outpatient and Inpatient Palliative Care, and GUIDE model participant
• Opportunity to enhance and expand services in the Cunningham Center for Compassionate Aging

Pay Range

$250,016 - $280,010 /year
