The position emphasizes documentation, collaboration, and compassionate, patient-centered care within a facility setting and travel across the assigned service area.

Qualifications

• Must be at least 18 years old.
• 3+ years of relevant field experience or a Bachelor's/Master's degree.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout the assigned service area.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician.
• Develop service plans and treatment plans for individuals or groups.
• Offer linkage, interventions and crisis prevention services to restore daily living skills.
• Collaborate with care teams and maintain professional documentation.
• Foster positive relationships with outside contacts and stakeholders.
• Demonstrate clinical, communications and documentation competencies.
