Job summary

The Human Service Forum is seeking a full-time case manager to support the Senior Care Option (SCO) program within the Primary Care Team in Holyoke, MA. The role is 35 hours per week with hybrid work up to 3 days after training.

You will assess needs, coordinate and monitor services, and advocate for public benefits to maintain health and independence while preventing premature institutionalization. Spanish/English fluency is required.

Qualifications

• BA/BS in Social Work, Human Services or related field
• Experience in case management with geriatric population
• LSW a plus
• Must have valid driver’s license/transportation
• Spanish/English fluency required
• Must be able to climb multiple flights of stairs

Responsibilities

• Function as a case manager within the Primary Care Team for the Senior Care Option program
• Coordinate, implement, and monitor contracted and community services to improve health and functional status
• Conduct periodic reassessments to determine appropriateness of the service plan and ensure access to public benefits

Responsible for functioning as a case manager within the Primary Care Team (PCT) for members participating in a Senior Care Option (SCO) plan. Make recommendations, coordinate, implement, and monitor contracted and community services for the purpose of improving or maintaining the member’s overall health and functional status. Conduct periodic reassessments to determine appropriateness of the service plan and ensure that members have access to public benefits to support their wellness, independence, with the goal of preventing premature institutionalization.

Travel reimbursement is $0.725/mile

Minimum Qualifications: BA or BS in Social Work, Human Services or related field, experience in case management with geriatric population. LSW a plus. Must have valid driver’s license and/or reliable transportation. Spanish/English fluency required. Must be able to climb multiple flights of stairs.
