Job summary

Old Colony Elder Services (OCES) seeks a dedicated Geriatric Support Service Coordinator (GSSC) for the Senior Care Options department to determine eligibility and ensure the provision of appropriate services to eligible members.

You will assess health and functional status, develop care plans, and maintain ongoing coordination with consumers and their families, including home visits and documentation. Bilingual Chinese proficiency is preferred to support diverse clients.

Qualifications

• Two years of experience in the human services field
• Bachelor's degree in social work or related field
• Current licensure if appropriate
• Bilingual with Chinese (Cantonese or Mandarin) preferred
• Excellent written and verbal skills

Responsibilities

• Promote independent functioning of members and provide services in the least restrictive environment
• Perform initial and ongoing assessments of health and functional status
• Develop individualized care plans and coordinate with care team
• Provide regular contact with consumers, including home visits
• Maintain accurate documentation
