Job summary

Boston Senior Home Care is seeking a SCO Bilingual GSSC (Mandarin/Cantonese) to assess elders' needs, determine eligibility for services, and create client-centered service plans. Travel up to 70% is required, with home visits and meetings as part of the role.

Under the Team Manager, you will document regularly, coordinate with SCO representatives, and advocate for clients while providing ongoing support and linkage to community resources.

Qualifications

• Bachelor's Degree required.
• LSW experience preferred, but not required.

Responsibilities

• Perform initial and ongoing assessments of client needs including eligibility for BSHC services.
• Develop service plans based on assessments to address identified needs.
• Maintain complete, accurate client records and regular documentation.
• Engage with client, family, and external supports; coordinate with SCO representatives and vendors.
