Job summary

Pine View Nursing seeks a Speech-Language Pathologist to evaluate and treat residents with speech, language, cognitive, and swallowing disorders in a skilled nursing facility. You will develop individualized therapy plans and collaborate with an interdisciplinary team to optimize communication and swallowing outcomes.

You will document progress, educate patients and families, and contribute to quality-of-life improvements for geriatric care across the care continuum.

Qualifications

• Graduate degree in Speech-Language Pathology from an accredited program.
• CCC certification.
• Strong clinical knowledge and experience in adult/geriatric care.

Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing (dysphagia) disorders.
• Develop and implement individualized therapy plans to meet rehabilitation goals.
• Collaborate with nurses, therapists, dietitians, social workers to create a comprehensive care plan.
• Maintain thorough records of assessments, treatment plans, progress notes, and discharge summaries.
• Educate residents and families about therapy goals, strategies to maximize quality of life.
• Provide support and education for end-of-life communication and swallowing needs.
