Job summary

Clove Lakes Health Care and Rehabilitation Center in Staten Island seeks a Speech Language Pathologist (SLP) to provide diagnostic and therapeutic services for residents with swallowing or communication impairments. The role participates in interdisciplinary meetings and helps develop policies to prevent decline in communication and swallowing.

The position requires NY licensure, ASHA CCC-SLP or equivalent, and strong computer/EMR skills.

Qualifications

• A current license as a Speech Language Pathologist in NY.
• ASHA CCC-SLP certificate or equivalent credentials.
• Equivalent education/work requirements for certification and pursuing ASHA examination.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Evaluate residents’ functional needs as referred by physician or interdisciplinary team.
• Follow physician orders and facility policies to evaluate/treat residents and document care.
• Assist the DON with communication/swallowing prevention policies and procedures.
• Participate in discharge planning when appropriate.
• Perform diagnostic tests and document evaluations per policies.
• Provide treatment to maximize communication/swallowing function.
• Recommend treatments, prevention measures, and assistive devices.
• Educate residents and caregivers to maintain/improve communication/swallowing.
• Maintain timely resident reports for regulators and payers.
• Perform duties effectively, timely, and professionally.
