Job summary

LongTerm Healthcare is seeking a dedicated Speech-Language Pathologist (SLP) for a standalone Skilled Nursing Facility in Lockport, NY. The role focuses on evaluated, diagnosing, and treating residents with speech, cognitive, and swallowing disorders, while collaborating with a multidisciplinary team.

You will develop personalized treatment plans and educate families and caregivers on progress, documentation, and regulatory compliance in a resident-centered environment.

Qualifications

• Current New York Speech-Language Pathologist license.
• CCC-SLP preferred; CFY candidates may be considered.
• SNF/geriatric care experience is a plus but not required.

Responsibilities

• Evaluate and treat residents with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on each resident's goals and needs.
• Collaborate with physicians, nursing staff, rehabilitation professionals, and families to deliver patient care.
• Maintain accurate documentation and ensure regulatory compliance.
• Educate residents and caregivers on treatment strategies and progress.
