Job summary

The Ridge Post-Acute in San Jose, CA is seeking a compassionate Speech Language Pathologist to join our skilled nursing facility team. You will conduct assessments, develop individualized treatment plans, and provide interventions to improve speech, language, swallowing, and cognitive skills for our residents.

Collaborate with therapists and nursing staff, educate families, and participate in discharge planning.

Qualifications

• New passer/graduates and CF supervision are welcome
• Previous experience as Speech Language Pathologist (SLP) or Speech Therapist (ST) is a plus
• Skilled Nursing or Long-Term Care experience is a plus
• CCC-SLP certification is required
• State licensure as SLP is required
• Interest in working with geriatric population
• Compassionate and patient-centered approach to care
• Ability to adapt to changing priorities in a fast-paced environment

Responsibilities

• Conduct comprehensive assessments to evaluate patients' speech and language abilities.
• Develop and implement individualized treatment plans targeting speech, language, voice, swallowing, and cognitive goals.
• Provide therapeutic interventions including articulation, language development, voice therapy, and cognitive training.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with OTs, PTs, and nurses to coordinate patient care.
• Educate patients, families, and caregivers about speech and language disorders and home exercises.
• Participate in team meetings and discharge planning discussions.
