Responsibilities

• Develop and implement individualized treatment plans based on assessment findings.

• Monitor and document residents' progress and adjust plans as needed.
• Ensure compliance with regulatory standards and facility policies.
• Maintain a clean, organized, and safe therapy environment.
• Collaborate with rehabilitation team to tailor treatment plans.
• Instruct patients and families in home programs to meet goals.
