Job summary

Modesto Post Acute Center is seeking a Speech Language Pathologist (SLP) to manage and direct the nursing department's speech therapy services. You will assess residents, develop individualized plans, educate families, and monitor progress with a multidisciplinary team.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current state licensure or certification as a Speech-Language Pathologist (SLP).
• CCC-SLP credential preferred.
• Experience in a skilled nursing facility or similar healthcare setting preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct comprehensive assessments of residents' speech, language, voice, and swallowing functions.
• Develop and implement individualized treatment plans based on assessment findings.
• Educate residents and families on communication strategies and techniques.
• Monitor and document residents' progress, adjusting plans as necessary.
• Ensure compliance with regulatory standards and facility policies.
• Maintain a clean, organized, and safe therapy environment.
• Collaborate with rehabilitation team to develop appropriate treatment plans.
• Develop home programs and train families to support therapy goals.
