Job summary

Intersect Healthcare, Inc. in Michigan seeks a Social Service Manager to oversee psychosocial services for residents and to assess emotional and behavioral issues related to illness.

You will participate as part of an interdisciplinary team and may assist with treatment planning and discharge planning. The role requires a bachelor’s degree in social work, MI licensure as a social worker, and at least one year of elderly social service experience, with strong supervisory and coordination skills

Qualifications

• Requires a bachelor’s degree in social work and at least one year of experience in a social service program for the elderly or related field.
• Must be licensed (full or limited) as a bachelor’s level clinical social worker with the State of Michigan.
• Ability to work with an interdisciplinary team and coordinate with community resources.

Responsibilities

• Assess and evaluate residents' psychosocial needs and develop goals for services.
• Attend care planning conferences and discharge planning as team lead.
• Assist residents and families with community resources and resolution of grievances.
• Maintain confidential records and supervise social work interns.
• Coordinate services with psychiatric providers and OBRA requirements.
• Support staff education and participate in marketing and program development.
