Carease Health in PA is seeking a compassionate Social Worker/Care Companion for part-time work in skilled nursing facilities. You will provide emotional support, brief therapy and psychosocial assessments as part of a Collaborative Care team.

You’ll collaborate with psychiatric providers, document visits meticulously and schedule-flexible hours that fit your day. This role emphasizes resident-centered care and ongoing professional supervision.

Qualifications

• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree.

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
