Job summary

Westminster Towers Orlando is seeking a Social Worker to join our Health Center team. You will assess residents' emotional and social needs, develop psychosocial care plans, and support individuals and families through admissions and ongoing care.

This full-time position includes a Monday–Friday 8am–5pm schedule in Orlando, Florida. You will coordinate with community resources, assist with discharge planning, advocate for residents, participate in QA activities, and monitor psychotropic

Qualifications

• Bachelor's degree in Social Work, Sociology, Healthcare Administration, or related field.
• One (1) year experience in a skilled nursing facility or medically related area.

Responsibilities

• Prepare comprehensive psychosocial assessments and care plans for all residents.
• During the Admission process, tour facility and guide potential residents and family members, reviewing and obtaining signatures for required documentation.
• Resident and family outreach – advocate for residents and contact family members.
• Access to community resources and present residents/family with healthcare provider options (home health, hospice, equipment providers, etc.).
• Assist with discharge planning; ensure appropriate discharges to a less restrictive setting.
• Assist with resolving resident/family grievances; conduct investigations and follow ups with resolutions.
• QA and audits; monitor psychotropic medications and mood/behavior symptoms; obtain consent for psych med use and maintain binder.

Education
