Self-Help For The Elderly in San Francisco, CA seeks a Social Worker to assess the social, emotional, and economic needs of Adult Day Services’ participants and provide comprehensive social services to meet those needs.

The role is regular part-time with non-exempt status, reporting to the Director, and requires a bachelor’s or master’s degree in social work with related aging/long-term care experience, bilingual English-Cantonese, and valid TB clearance.

Formación

• Bachelor’s degree in social work with two years of experience in aging/long-term care.
• Master’s degree in social work, psychology, gerontology, or counseling with at least one year in aging/long-term care.
• Bilingual in English and Cantonese; Mandarin a plus.

Responsabilidades

• Assess participants' social, emotional, and economic needs and provide social services.
• Conduct home visits for intake and enrollment paperwork.
• Coordinate transportation for assessments and IPC scheduling.
• Develop and monitor Individual Plans of Care (IPCs).
• Provide counseling and liaison with caregivers and multidisciplinary team.
• Promote peer support groups and caregiver groups.
• Document services and progress; conduct 6-month reassessments.
• Discharge planning and maintain community agency relationships.
