Job summary

Acacia Network is seeking a Geriatric Social Worker to provide case management, counseling, advocacy, and resource coordination for older adults and their families. The role requires independent judgment, confidentiality, and a commitment to enhancing well-being.

You will assess needs, develop service plans, monitor progress, and collaborate with healthcare providers and partners to connect clients with housing, healthcare, and community resources. A MSW and NY LMSW/LCSW licensure are required.

Qualifications

• Master's degree in social work (MSW) from an accredited college or university.
• Current NYS Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required and must remain in good standing.
• Minimum of two (2) years of experience working with older adults, case management, mental health services, or related human services programs preferred.

Responsibilities

• Provide social work and case management services to older adults and their families.
• Conduct assessments, develop service plans, and monitor participant progress.
• Provide counseling, referrals, advocacy, and crisis intervention as needed.
• Assist participants in accessing benefits, housing, healthcare, transportation, and community resources.
• Maintain accurate case records and enter documentation into required electronic systems.
• Develop and facilitate workshops and activities that support mental health and wellness.
• Provide monthly reports and quality assurance metrics as required.
• Collaborate with staff, healthcare providers, and community partners.
• May be required to assist and provide coverage at other Older Adult Centers operated by the organization on an as-needed basis.
• Perform other duties as assigned.
