Job summary

CareRite Centers, LLC is seeking a dedicated Social Worker for a full-time role in a Skilled Nursing Facility. The position focuses on addressing residents’ psychosocial needs, coordinating care plans, and guiding transitions from admission to discharge.

The role requires a BSW (MSW preferred) with strong communication skills and experience in SNF settings. Proficiency with EMR systems and PCC/MDS 3.0 is a plus.

Qualifications

• BSW required; MSW preferred.
• Experience in SNF, rehab, or long-term care preferred.
• Strong communication and interpersonal skills.
• Familiarity with discharge planning and community resources.
• Ability to manage multiple responsibilities in a fast-paced environment.
• Familiarity with PCC and MDS 3.0 is a plus.

Responsibilities

• Evaluate residents’ social, emotional, and psychological needs.
• Develop and implement individualized care plans in collaboration with the clinical team.
• Participate in interdisciplinary care plan meetings.
• Coordinate admissions, transfers, and discharge planning.
• Assist with arranging home care services, DME, and community resources.
• Provide counseling and support to residents and families.
• Ensure residents’ rights are maintained and advocate for their well-being.
• Assist with financial documentation and educate residents/families on resources.
• Maintain accurate and timely documentation per regulatory standards.
