Job summary

Allison Care Center seeks a Social Services Specialist to support residents and families through placement, planning, and transitions, ensuring care aligns with federal/state regulations and company policies. You will collaborate with the Social Services Director to develop psychosocial histories and care plans upon admission, quarterly reviews, and changes in condition.

Qualifications

• Previous experience with geriatric population or long-term care setting preferred.
• High school diploma or equivalent required; AA or Bachelor's degree preferred.

• Ability to interact tactfully and professionally with staff, residents, families, visitors, government agencies, and the public.

Responsibilities

• Assists in interviewing and obtaining information from residents and families for social services history and care planning.
• Assists residents and families with transition and adjustment to placement in the facility.
• Assists with determining legal status on admission and handling advance directives per policy.
• Informs residents and families of their rights and options; follows policy for care decisions.
• Provides input on social, psychological, and emotional needs to supervisor.
• Ensures required evaluations and coordinates appointments and transportation.
• Supports residents and families in finding financial, legal, mental health, and community resources.
• Participates in resident and family council meetings as requested.
• Gains knowledge of regulations and reimbursement; assists with applications and eligibility requests.
• Supports theft/loss prevention program and works with Ombudsperson on rights questions.
• May perform documentation duties per charting and government regulations; ensures accurate and descriptive records.
• Assists in discharge planning and post-discharge orientation for residents and families.
