Job summary

White Oak Manor Tryon, part of NATIONAL HEALTH - WHITE OAK LLC, is seeking a caring and dependable Social Services Assistant to support residents and families in Tryon, NC. This full-time role collaborates with residents, nursing staff, and community partners to ensure a respectful, supportive stay.

You’ll handle admissions, care planning, and discharge planning, complete social histories and notes, coordinate appointments and transportation, and participate in the interdisciplinary care team

Qualifications

• Bachelor’s degree in Social Work or related Human Services field.
• Strong communication, organization, and follow-through.
• Ability to handle sensitive situations with patience, professionalism, and compassion.
• Team player who can coordinate with departments and outside providers.

Responsibilities

• Support residents and families with social service needs throughout their stay.
• Assist with admissions, care planning, and discharge planning.
• Complete resident social histories, progress notes, and other required documentation.
• Communicate with residents, families, nursing staff, hospitals, providers, and outside agencies.
• Help residents and families work through concerns, changes, and difficult situations.
• Coordinate medical appointments, transportation, equipment, and other resident needs.
• Participate as a member of the interdisciplinary care team.
• Report changes in resident condition or concerns to the appropriate team members.
• Maintain resident confidentiality and follow HIPAA, facility, state, and federal requirements.
