Job summary

Orchard Health & Rehab seeks a Social Services Director to lead the Social Services Program, coordinating activities that promote patient well-being and family involvement. You will counsel patients and families, manage admissions/discharges, and liaise with community agencies to support high-quality care.

The role requires organization, communication, and supervisory experience in a care setting, with preference for a social work degree.

Qualifications

• Bachelor degree preferred in social work or related field.
• Minimum two years supervisory experience in a hospital, skilled nursing center, or medical center.

Responsibilities

• Plan, develop, organize and direct the Social Services Program.
• Coordinate and participate in family and patient activities to promote social interaction.
• Counsel patients and families including dealing with emotions and limitations.
• Complete the social service portion of the MDS within 14 days of admission.
• Coordinate admissions and discharges.
• Act as liaison with social, health, and community agencies.
