Qualifications

• Bachelor's degree in social work or related human services field.
• State Clinical Social Work Licensure.
• Health care social work experience preferred.
• Ability to communicate effectively with residents and families.

Responsibilities

• Coordinate social services and discharge planning within the hospital system.
• Develop measurable plans to improve social service operations.
• Perform psychosocial assessments and reassess residents.
• Ensure resident safety and reduce care errors.
• Involve families in teaching and care planning.
• Interact professionally with residents and families.
• Provide resources and support to Social Service/Discharge Planning staff.
• Maintain good rapport with medical staff and other departments.
• Lead and manage the Social Service/Discharge Planning Department.
