Job summary

Abington Manor is seeking a Social Service Coordinator to support residents in a nursing home setting. The role focuses on psychosocial assessment, arranging services, and advocating for residents’ rights and well-being.

The ideal candidate holds a Bachelor’s in Social Work or related field, with experience in elderly populations and a strong ability to collaborate with a multidisciplinary team. The position offers full-time hours and opportunities for growth within the facility.

Qualifications

• Bachelor's degree in Social Work, sociology, special education, rehabilitation counseling, psychology, or related field required.
• Minimum three years supervised social work experience in a health care setting preferred.
• Prior work with elders in a community or long-term care setting preferred.
• Ability to communicate effectively with residents, families, and multidisciplinary teams.

Responsibilities

• Assist to the Director of Social Services and identify residents' psychosocial, mental and emotional needs.
• Develop and aid in access to services to meet resident needs.
• Exercise professional judgment to maximize resident well-being and quality of life.
• Perform administrative duties, advocacy and care planning, and coordinate referrals and admission processes.
