Qualifications

• Bachelor's degree from an accredited college or university.
• Knowledge of the medical case management process, needs of geriatric patients, health care settings, and funding resources (Medicare, Medicaid, private insurance).
• Imagination and creative ability; ability to work cooperatively with other professionals and organize responsibilities effectively.

Responsibilities

• Work with individual patients and families toward adjustment to center life and to crises of illness, disability, and death.
• Advocate for patients and families in expressing, defining, and resolving grievances.
• Report to the Director of Social Services the current status of social services and present needs.
• Participate in developing patient care plans in cooperation with individuals from other disciplines.
• Maintain confidentiality of professional information.
