• Knowledge of the medical case management process and funding resources (Medicare/Medicaid, private insurance).
• Ability to organize and carry out responsibilities efficiently and effectively.
• Insurance authorization experience preferred.

Responsibilities

• Coordinate discharge planning functions with care teams.
• Advocate for patients and families to express and resolve grievances.
• Report the status of social services to the Director and identify needs.
• Participate in developing patient care plans with other disciplines.
• Maintain confidentiality of professional information.
