Qualifications

• Must be board certified in Internal Medicine, Family Medicine, or have a Geriatrics fellowship.
• Experience in managing complex geriatric patients.
• Ability to communicate effectively with family members regarding care decisions.

Responsibilities

• Provide clinical care for inpatients in a skilled nursing facility.
• Serve on interdisciplinary care teams.
• Facilitate daily care team meetings.
• Discuss care goals with patients and families.
• Oversee Nurse Practitioners providing care services.
