Hope Creek Nursing and Rehabilitation Center seeks a dedicated SLP Speech Language Pathologist to provide exceptional care at our facility in East Moline, Illinois. You will assess, treat, and manage communication and swallowing disorders, develop individualized treatment plans, and educate residents and staff.

Qualifications

• Master's degree in Speech Language Pathology from an accredited program.
• Clinical Fellowship (CF) completed.
• Illinois Speech Language Pathology license.

Responsibilities

• Conduct initial evaluations and continued assessments of residents' communication and swallowing disorders.
• Develop and implement individualized treatment plans to address residents' specific needs.
• Provide treatment sessions, including speech therapy, language therapy, and swallowing therapy.
• Educate and train residents, families, and staff on communication and swallowing techniques.
• Collaborate with other healthcare professionals, including physicians, occupational therapists, and physical therapists.
• Maintain accurate and detailed records of resident assessments, treatments, and progress.
• Stay current with industry developments and best practices through ongoing education and professional development.
