Job summary

H2 Health is seeking a Speech-Language Pathologist to provide 1:1 care for geriatric patients in outpatient settings. The role emphasizes dysphagia management, independent judgment, and collaboration with an interdisciplinary team in a flexible full-time schedule.

Prior outpatient therapy experience and a CCC-SLP state license are required. This is a clinician-led organization offering mentorship, professional growth, and a broad network across states.

Qualifications

• Education: Graduate of an accredited Speech Language Pathology program.
• Experience: Prior outpatient therapy is a plus; dysphagia experience is required.

Responsibilities

• Evaluate and treat a primarily geriatric caseload 1:1.
• Work independently and in teams to decide based on patients\' functional needs.
• Collaborate with an interdisciplinary team to deliver therapy opportunities.
