Job summary

American Medical Associates in Brooklyn, NY is seeking an experienced RN Case Manager-ALP to support residents in an assisted living program. You will oversee resident care, supervise staff, collaborate with physicians, process orders, and document case notes.

You will also ensure residents receive their 6- and 12-month medical evaluations and serve as the on-site contact for operational issues when administration is unavailable.

Qualifications

• Must have a valid New York RN license.
• UAS certification and experience completing UAS assessments strongly preferred.
• Must have prior experience as a Case Manager.
• Experience in a geriatric setting, preferably an assisted living program.
• Strong written and verbal communication skills.

Responsibilities

• Take responsibility for resident care and staff supervision.
• Work closely with MD and pick up MD orders.
• Write case notes and ensure all residents have their 6 and 12 month medical evaluations.
• Act as the on-site point of contact for operational issues when administration is unavailable.
