Choice in Aging seeks a Registered Nurse Care Manager (RNCM) under MSSP to assess and coordinate services for frail low-income elders enrolled in the MSSP Medi-Cal Waiver. Under the supervision of the Multipurpose Senior Services Program (MSSP) Site Director/Supervising Care Manager, the Registered Nurse Care Manager (RNCM) functions as a geriatric care manager for frail low-income elders enrolled in the MSSP Medi-Cal Waiver program. The RNCM evaluates the potential client as a total person and identifies the functional and health limitations that impede independent living. They recommend, provide, and coordinate a wide range of social and health services to assist MSSP participants in maintaining safe community living. The MSSP Registered Nurse Care Manager works as a member of the Choice in Aging (CiA) team, providing health and social services to frail elders and adults with disabilities to meet CiA's mission to create opportunities where people can learn, grow, and age independently with dignity in community.
