Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to deliver treatment under the supervision of an Occupational Therapist. The COTA will implement goals set by the OT and document patient progress in a timely manner.

Experience in long-term or geriatric care is preferred, with emphasis on high-quality patient-centered service. Responsibilities include collaborating with all disciplines, maintaining professionalism, and reporting to the Director of

Qualifications

• Must have a valid professional license and registration for state of practice.
• Provide comprehensive treatment to patients under supervision of Occupational Therapist
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Document patient’s progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best practice treatments under supervision of the OT
• Provide treatment adhering to goals developed by the OT
• Provide feedback to supervision OT via documentation or as required by policy
• Collaborate with disciplines to ensure patient-centered care
• Maintain punctuality and professional conduct
