Job summary

Chatham Hills Subacute Care Center is seeking a Certified Occupational Therapy Assistant (COTA) to deliver patient-centered treatments under the supervision of an Occupational Therapist. You will follow OT-designed goals, document weekly progress, and collaborate with the rehab team to ensure high-quality care for residents in a long-term care setting.

The role emphasizes timely progress notes, professional conduct, and dependable scheduling.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.
• Document patient progress on weekly progress notes in a timely fashion.
• Collaborates with all disciplines to provide best patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out best-practice treatments under the supervision of the OT.
• Provide treatment per goals developed by the OT.
• Provide feedback to supervising OT via documentation or facility policy.
