Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team, supporting seniors in skilled nursing and long-term care settings. The PTA will implement individualized treatment plans designed by the Physical Therapist, focusing on functional mobility, safety, and independence.

You'll collaborate with an interdisciplinary team in a supportive, patient-centered culture grounded in dignity, compassion, and respect.

Qualifications

• Licensed Physical Therapist Assistant with active state credential.
• CPR certification required.
• Experience with SNF/long-term care settings preferred.

Responsabilités

• Implement individualized treatment plans designed by the PT.
• Conduct gait and balance training and therapeutic exercises.
• Assist with transfers and functional mobility activities.
• Provide patient and family education and support.
• Document timely and accurate EMR entries.
• Collaborate with an interdisciplinary care team.
