Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant to provide in-home geriatric therapy in St. Augustine, FL.

The PRN role offers flexible assignments with pay set at $65 per 4-unit treatment, enabling clinicians to choose visits based on availability. The PTA will work under the plan of care to help residents maintain mobility and independence, communicating with physicians and families, and documenting care accurately.

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program.
• Active Florida PTA license.
• Two years of clinical experience preferred (not required).

Responsibilities

• Provide skilled physical therapy treatments under the plan of care.
• Help patients improve mobility, function, and independence.
• Communicate effectively with physicians, families, and the care team.
• Complete accurate, timely documentation.
