Job summary

AdviniaCare Providence is seeking a compassionate and skilled Physical Therapist to join our interdisciplinary team. You will design and implement personalized treatment plans to help residents achieve the highest level of physical, emotional, and cognitive function, promoting independence, safety, and quality of life.

Work closely with the rehabilitation team to assess needs, set goals, educate residents and families on exercises and techniques, monitor progress, and maintain accurate records

Responsibilities

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
• Maintain accurate and up-to-date records of patient care
