Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team. This on-site role focuses on implementing PT treatment plans for seniors in skilled nursing and long-term care settings, including therapeutic exercise, gait training, and safety-focused activities.

You will collaborate with an interdisciplinary team, document progress in EMR, and provide education to patients and caregivers.

Qualifications

• Must hold an active CPTA license.
• Experience in skilled nursing or long-term care settings preferred.
• Ability to implement PT treatment plans and document progress accurately.

Responsibilities

• Perform therapeutic exercises and gait/balance training.
• Assist patients with functional activities to improve mobility and safety.
• Document patient progress accurately in EMR and educate patients and caregivers.
