Job summary

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, offering on-site therapy to seniors in skilled nursing and long-term care settings.

You will implement PT treatment plans focused on mobility, safety, and quality of life, with responsibilities including therapeutic exercise, gait and balance training, functional activities, EMR documentation, and education for patients and caregivers.

Qualifications

• Certified PTA licensure required in the state of CT.
• Experience with elderly populations in skilled nursing or long-term care settings preferred.
• Ability to document accurately in EMR and educate patients/caregivers.

Responsibilities

• Implement PT treatment plans to improve mobility, function, safety and quality of life.
• Deliver therapeutic exercise, gait and balance training, and functional activities.
• Maintain accurate EMR documentation and patient education.
