Cordial Health, PC is seeking a Psychiatric Nurse Practitioner or Psychiatric Physician Assistant for an on-site, full-time role in Bronx, NY. You will conduct comprehensive psychiatric assessments and ongoing medication management for residents in nursing homes, with telehealth support as needed.

The position requires NY license with PMHNP/psychiatry PA certification or eligibility, strong collaboration with care teams, and a compassionate, trauma-informed approach to care for adults and older

Qualifications

• Current licensure and board certification (or eligibility) as a PMHNP or PA with psychiatry experience; active New York State license in good standing.
• Experience in psychiatric evaluation, diagnosis, and treatment planning for adults and older adults, preferably in nursing homes or long-term care.
• Proficiency in psychopharmacology and medication management for patients with comorbid conditions.
• Strong clinical documentation skills and comfort using EHR and telehealth platforms.
• Excellent interpersonal, communication, and collaboration skills with interdisciplinary teams and families.

Responsabilités

• Perform comprehensive psychiatric evaluations and ongoing medication management.
• Develop and update individualized treatment plans.
• Prescribe and monitor psychotropic medications and monitor safety and efficacy.
• Document encounters per regulatory and organizational standards.
• Collaborate with nursing home staff, primary care clinicians, and families to coordinate care.
