Qualifications

• Current licensure and board certification (or eligibility) as a PMHNP or PA with psychiatry experience; active New York State license in good standing.

• Proficiency in psychopharmacology and medication management for patients with comorbid conditions.
• Strong clinical documentation skills and comfort using EHR and telehealth platforms.
• Excellent interpersonal, communication, and collaboration skills with interdisciplinary teams and families.

Role Description

The Psychiatric Nurse Practitioner or Psychiatric Physician Assistant provides direct psychiatric evaluation and ongoing medication management to patients in nursing homes, with a focus on individualized, patient-centered care. Day-to-day responsibilities include conducting comprehensive psychiatric assessments, developing and updating treatment plans, prescribing and monitoring psychotropic medications, and documenting encounters in accordance with regulatory and organizational standards. The role involves close collaboration with nursing home staff, primary care clinicians, and families or caregivers to coordinate care, manage behavioral health crises, and support transitions of care. The clinician also participates in telehealth visits when needed, ensuring continuity of care and timely follow-up. This is a full-time, on-site role located in Bronx, NY.

Qualifications

• Current licensure and board certification (or eligibility) as a Psychiatric Nurse Practitioner (PMHNP) or Physician Assistant with psychiatry experience; active New York State license in good standing.
• Demonstrated experience in psychiatric evaluation, diagnosis, and treatment planning for adults and older adults, preferably in nursing homes, long-term care, or similar settings.
• Proficiency in psychopharmacology and medication management, including monitoring efficacy, side effects, and safety for individuals with comorbid medical conditions.
• Strong clinical documentation skills and comfort using electronic health records (EHR) and telehealth platforms in compliance with privacy and regulatory requirements.
• Excellent interpersonal, communication, and collaboration skills to work effectively with interdisciplinary teams, patients, and families in diverse communities.
• Ability to practice with a high degree of independence, sound clinical judgment, and ethical decision-making, while following evidence-based guidelines and practice protocols.
• Commitment to compassionate, non-judgmental care, cultural humility, and trauma-informed approaches to mental health treatment.
• Master’s or doctoral degree appropriate to NP role or graduate-level PA training from an accredited program; prior experience in psychiatry or behavioral health strongly preferred.
