Qualifications

• Active California license as NP or equivalent
• Compassionate, patient-centered care
• Strong communication and documentation skills
• Experience in geriatric populations preferred

Responsibilities

• Provide in-person psychotherapy and behavioral health services within SNFs and Assisted Living communities.
• Conduct diagnostic assessments and develop treatment plans.
• Treat depression, anxiety, adjustment disorders, grief, trauma in geriatric populations.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary care teams to improve patient outcomes.
• Build therapeutic relationships with residents and families.
